NOTICE OF PRACTICE POLICIES

I. APPOINTMENTS AND CANCELLATIONS

To secure a session, a valid payment method must be maintained on file. Valid payment methods include credit card or debit card. *In some cases, cash or check may be accepted at the start of the session. If a check is returned, the client will no longer have that option for future payment.  Returned checks will also be subjected to applicable bank and service fees.*

Payment for sessions will be processed no later than 15 minutes prior to the start of your session using the payment method on file. For example, if your session is at 1pm, your payment will be processed between 8am-12:45pm.  PAYMENT MUST BE RECEIVED BEFORE THE SESSION CAN BEGIN.

Canceled sessions will be subjected to a fee of the full session rate IF THE CANCELATION IS NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE of the session. This is necessary because a time commitment is made to and held exclusively for you.

The standard session time is 60 minutes. You can; however, request to change the length of your sessions. Requests to change the standard session timeframe need to be made in advance in order to adjust the schedule. If you are late for a session, you will lose that session time.

II. TELEPHONE ACCESSIBILITY

If you need to contact me between sessions, please leave a message on my voice mail at 706-903-9031. You can also email me at kiki@iamyourtherapist.com, or send me secure message through your client portal. I am often not immediately available; however, I will attempt to return your call or respond to your message within 24 business hours. Please note that we will not conduct sessions via phone calls. Phone calls should only be used to ask brief questions, and to provide or receive information. Any further decision will need to be done during a scheduled session. For emergencies, please call 911 or visit any local emergency room.

III. SOCIAL MEDIA AND TELECOMMUNICATION

Due to the importance of your confidentiality and minimizing dual relationships, I do not accept friend, follow, or contact requests of current or former clients from any personal social media/networking accounts, sites, or pages. I believe doing so will blur the boundaries of our working relationship, compromise your confidentiality, and jeopardize our respective privacy. You can; however, accept follow any of my professional social media/networking accounts, sites, and pages. If you have questions about this, we can talk more about it during your session.

IV. ELECTRONIC COMMUNICATION

I cannot ensure the confidentiality of any form of communication through electronic media, including emails and text messages. If you prefer to communicate via email or text messaging for scheduling or cancelations, I will do so; however, your secure client portal is preferable and available for such communication. While I will try to return messages in a timely manner, I cannot guarantee an immediate response and request that you do not use these methods of communication to discuss and/or request assistance for emergencies.

V. TERMINATION OF SERVICES

Ending relationships can be difficult, even working relationships. I will work to ensure that you are fully capable and ready to successfully operate your business prior to the end of your contracted timeframe. I may terminate our working relationship prior to the end of your contract if I determine that the service(s) provided are not being used for the intended purpose or if you are in default of payment. I will not terminate our working relationship, early or otherwise, without first discussing and exploring the reasons and purpose of termination with you.

Should you fail to schedule a session for four (4) consecutive weeks, unless other arrangements have been made in advance, I will consider our working relationship terminated.

INFORMED CONSENT FOR VIRTUAL SESSIONS

I. GENERAL INFORMATION

At times, it may be necessary to conduct virtual sessions. Virtual sessions can be requested and scheduled at any time during our work together.

Virtual sessions will not be the same as a direct face-to-face visit as we will not be in the same room.

Virtual sessions have potential benefits, including, but not limited to, easier access to care and the convenience of meeting from a location of your choosing.

Virtual sessions also have potential risks to technology, including, but limited to, interruptions, unauthorized access, and technical difficulties. Virtual sessions can be discontinued if video connections are not adequate.

II. CONSENT TO USE THE TELEHEALTH BY SIMPLE PRACTICE TECHNOLOGY SERVICE

Telehealth by Simple Practice is the technology service used to conduct virtual sessions. It is simple to use and there are no passwords required to log in.

Telehealth by Simple Practice is NOT an Emergency Service. In the event of an emergency, use a phone to call 911.

Though there will be direct virtual contact through the technology Service, neither Simple Practice nor the telehealth service provides medical, health, emergency or urgent care, advice or services. The Telehealth by Simple Practice service facilitates videoconferencing and is not responsible for the delivery of any health care, medical care, or advice.

The consultant does not have access to any of the technical information in the Telehealth by Simple Practice service and cannot attest to or verify that such information is current or accurate.

To maintain confidentiality, do not share your telehealth session link with anyone not authorized to attend the session.
