NOTICE OF PRIVACY POLICY

I. GENERAL INFORMATION

I understand the information you provide about yourself, and your business is personal. I am committed to protecting this information. I create a record of the services you receive from me in order to provide quality service and comply with certain ethical and organizational requirements. This notice applies to all of your records generated by YOUR THERAPIST COUNSELING SERVICES, LLC (YTCS), and will explain how I may use and disclose information obtained by you. It will also describe your rights to this information, and certain obligations I may have regarding the use and disclosure of the information.

II. HOW I MAY USE AND DISCLOSE INFORMATION ABOUT YOU AND YOUR BUSINESS

I pledge to ensure that all information about you and all aspects of your business is kept private. The following categories describe different ways that I may use and disclose your information. Not every use or disclosure in a category will be listed; however, all of the ways I am permitted to use and disclose information will fall within one of the categories:

1.      If you are utilizing my business consulting services by way of a school, company, or organization, I may be obligated under the terms and conditions set by the aforementioned to provide certain information about your business, as well as an assessment of your engagement, progress, and potential success of your business.

2.      To promote and/or market aspects of your business on your behalf through various methods including, but not limited to, my social media pages, direct interaction and engagement with interested and relevant third parties, or written communication with interested and relevant third parties.

3.      To promote and/or market aspects of YTCS through various methods including, but not limited to, my social media pages, written or video testimonials and reviews, direct interaction and engagement with interested and relevant parties, or written communication with interested and relevant third parties. 

4.      To advocate for and solicit needed resources, services and/or referrals for the wellness and growth of your business. 

5.      To broker connections with other businesses, community organizations, and industry leaders.

6.      To compile statistics on success rates, timelines of progress, business scaling, and continued success. 

III. USES AND DISCLOSURES THAT DO NOT REQUIRE YOUR AUTHORIZATION

Subject to certain limitations in the law, I can use and disclose your information without your authorization for the following reasons:

1.      When disclosure is required by state or federal law, and the use or disclosure complies with and is limited to the relevant requirements of such law.

2.      For public health and mandated reporting requirements, including reporting suspected child, elder, or dependent adult abuse, or preventing or reducing a serious threat to anyone’s health or safety.

3.      For audits and investigations.

4.      For judicial and administrative proceedings, including responding to a court or administrative order, although my preference is to obtain an authorization from you before doing so.

5.      For law enforcement purposes, including reporting crimes occurring on my premises or that I witness.

6.      To coroners or medical examiners, when such individuals are performing duties authorized by law.

7.      For research purposes, including, but not limited to, studying and comparing best business practices, consulting methods, resources, and technology.

8.      For appointment reminders and continuity of services. I may use and disclose your personal information to members of my staff to remind you of a session, encourage the continuation of remaining contracted sessions or services, and/or notify you of outstanding/overdue payments.

IV. USES AND DISCLOSURES THAT YOU CAN OBJECT

I may provide information to a family member, friend, or other person(s) that you indicate, in writing, is involved in your business or the payment for your services, unless you object in whole or in part. The opportunity to consent may be obtained retroactively in emergency situations.

V. YOU HAVE THE FOLLOWING RIGHTS WITH RESPECT TO YOUR PERSONAL AND BUSINESS INFORMATION

1.      The right to request limits on uses and disclosures of your information. You have the right to ask me not to use or disclose certain information for consulting services, payment, or normal operation purposes. I am not required to agree to your request and may deny/decline your request if I believe it will affect our work together.

2.      The right to choose how I send information to you. You have the right to request a specific method of contact, such as home, cell or office phone, or to send mail to a different address. I will agree to all reasonable requests.

3.      The right to see and get an electronic or paper copy of information I have about you and your business. I will provide you with a copy of your record or a summary of it, if you agree to receive a summary, within 30 days of receiving your written request. I may charge a reasonable, cost-based fee for doing so.

4.      The right to get both a paper copy and email of this notice.
